 Notice to Patients of Preserve Health, P.A. Medical Practice
We are not providing clinical care at this time.
This includes office visits, labs and prescriptions.
We are sorry for the inconvenience   

Our new hours are Tuesday, Wednesday and Thursdays
8:30-12noon.

Your records are still available and can remain with us until you need them.  A signed medical release is required for records if you do wish to have them. 

We are providing records on CD for a charge of $15.




I will soon be transitioning to other personal and professional goals and will be ending my employment with the Preserve Health medical practice.  My final day of providing medical care at Preserve Health will be Friday, December 9, 2011.  I will not be able to attend to you professionally after that date.

It is my understanding that The Cliffs is recruiting a new physician for a medical practice in the community and has a plan to continue to provide a medical practice for the benefit of the community.  They will make an announcement about the future of a medical practice at The Cliffs in the next several weeks. 

Because I am leaving Preserve Health medical practice, we need to arrange for the transfer of your medical records at your request if you choose.  Because the records of your medical care are confidential, I will require your written authorization to make them available to another physician.  If you choose, I will provide a copy of your medical records to the physician that you designate after I have received your authorization.  As an alternative, you may choose to pick up from Preserve Health a copy of your record in an electronic format after we have received your signed authorization.  There will be a $15 charge for the administration of this electronic record management process.  Paper hard copies will have an additional charge.

You may keep your records at Preserve Health while its future is made clear and no release will be necessary unless you transfer out of the practice.

I am sorry that I cannot continue as your physician.  I extend to you my best wishes for your future health and happiness.
