If you are not pre-registered for your appointment via www.PreserveHealth.com or by telephone, we strongly encourage patients to arrive for their first appoint approximately 15 minutes early in order to complete the registration process. Please bring your health insurance cards with you at this time to assist in the registration and billing process. At subsequent visits, you will be asked to verify your personal information and you should notify a Preserve Health team member of any changes. Also, remember to bring all of your medication bottles, including supplements.
