Who will complete my insurance forms or write letters I might need?
	Your provider will complete medically-related forms and write letters on your behalf upon request. Please allow 10 days for your request to be processed. A reasonable fee will be charged for this service. Some forms require information that can be completed only by having an appointment with the provider, therefore you will be asked to schedule an appointment in order that the information can be gathered and the form completed.
