If your Preserve Health provider determines that you need to be hospitalized, we will decide together to which consultant and to which facility you will be referred.  If you are admitted to any hospital, be sure to let your doctor know that you are a patient of Preserve Health and ask that a copy of your hospital record be sent to us.
